Purpose of Your Position To plan, implement and evaluate nursing care provided, including restorative care for residents. As the direct care provider of health services, he/she shares appropriate information with all other health services staff in an effort to assist each resident in achieving his/her maximum level of development.    Position Functions and Responsibilities - Promotes Elizabeth Seton Children’s Mission and Core Values of Respecting and Embracing Diversity, Excellence, Collaboration, Innovation, Evolution, Growth and Change and Loving Relationships by bringing these values to life in the organization. - Utilizing the nursing process, the RN assesses the resident, plans intervention and evaluates the outcome of care provided. - Performs treatments and special care as required, ex. tracheotomy care, central venous lines, oxygen administration, cast care, dressing changes, etc. - Administer medication and immunization as ordered. - Confer with physician or other health team members regarding patient care management. - Assist the physician in performing physical examinations and treatments. - Evaluate the effects of prescribed treatment including orthotics and report same to appropriate disciplines. - Observe, report and record abnormal vital signs, seizures activity, other disease manifestations and other clinically significant data. - Implement plan of care as outlines in the resident’s Interdisciplinary Care Plan including positioning, play, feeding techniques, behavior modifications plan etc. - Participate in completing MDS. Revised care plan as resident condition changes. - Assumes the role of nurse in charge of a unit for the shift as assigned - Educate residents and families with regard to their treatment plan - Document interventions and teaching outcomes accurately and timely - Maintain a safe therapeutic resident care environment. - Directs and supervises CNAs, unit a clerk, LPN and other RN’s when in charge. - Establishes effective working relations with staff residents and family members. - Ensures that the Center and the Department of Nursing policies and procedures are follow in the planning implementation and evaluation of care. - Review medication administration records for completeness at end of shift. - Give and receives the nursing report and follow through on any special instructions. - When in charge makes periodic checks to ensure that prescribed treatments are being properly administered by LPN’s and CNAs and to evaluate the residents physical and emotional status. - Participates in the orientation of new staff residents and families to the unit. - Carries out restorative and rehabilitative programs including teaching resident to wash/feed/dress self as appropriate. - Ensures that the facility policy on the use of restraints is followed and that documentation is timely and accurate. - Ensure that the unit is kept clean free from clutter and that the supply room door is kept locked; request the supervisor’s intervention as necessary to achieve this. - Follows infection control standards and ensures that assigned personnel do likewise. - Keep abreast of new nursing policies and procedures. - Keep nurses notes, plan of care and all other documentation pertaining to your assigned resident timely accurate and complete. - Carries out end of shift responsibility e.g. supplies are available for next shift’. Unit is clean and tidy; all documentation is complete and accurate. - Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator.

Monday-Friday 10a-6p  After 6 month probation period option for (4) day 8a-6p schedule   In this role, the Physical Therapist will provide evaluation and individual treatment to assigned residents. Complete all necessary documentation i.e. evaluations, progress notes, discharge summaries, adaptive equipment justification letters, etc.) Attend and report at all care plan meetings. Provide clinical supervision to PT students. Featuring the largest long term residential medical center of its kind in the United States.   This is a Per Diem Pharmacist opportunity- coverage needed within Monday-Friday 10a-6p   The Pharmacist will provide pharmaceutical services in accordance with federal, state, local standards and the policies and procedures of the Pharmacy department of the Pediatric Center. Responsibilities will include assisting the Director of Pharmacy in providing quality pharmacy services to all the residents of the Pediatric Center. Including but not limited to:   - Fill and dispense prescriptions. - Provide consultation on all aspects of pharmacy services and medication information to staff, residents and families. - Profile patient orders to computer system. - Print accurate physicians’ order sheets and medication administration records. - Properly stores medications, chemicals and biologicals. - Assist Director in the ordering of medications, chemical and biologicals. - Assist in the maintenance of inventory by removing expired medications and rotating stock. - Inspect nursing units and reports problems to Pharmacy Director. - Package medication for use on the nursing units. - Pick up orders and deliver medications to the nursing units. - Participate in department quality improvement activities as indicated. - Compound medications using special formulations in pharmacy compounding book and keep records of compounded prescriptions. - Replenish floor stock medications for nursing units and other departments. - Assume responsibility for the operation of the department in the absence of the director. - Demonstrate knowledge of the pharmacy’s and Pediatric Center’s policies and procedures. - Verify and replace medication orders for medications provided from the night cabinet by the nursing supervisor when the pharmacy is closed. - Maintain a communication log on all consultations regarding medication orders and food-drug interactions. - Must have at least 45 hours of continuing education completed every 3 years. Featuring the largest long term residential medical center of its kind in the United States.   This is a full time Monday-Friday 40 hour work week with rotating late night Thursday until 6p.     Our Social Workers provide medically related social work services, which support the physical, mental and psychosocial well-being of each resident and family.   Responsibilities will include: - Assists families with the pre-admission process, facility tours, and Admissions paperwork. - SW orients the family to the Pediatric Center and facilitates the admission process with the interdisciplinary team. SW reviews the Admission Packet and the Resident Bill of Rights. - SW assists resident/family with adjusting to the Pediatric Center and acts as a liaison between the Center and resident/ family throughout their stay. - SW interviews the family and any collateral contacts to complete the initial psychosocial assessment and updates as needed. - SW assesses and assists the resident and family with governmental benefits, such as SSI and Medicaid as well as school placement, guardianship and other legal/social needs. - Participates as an integral member of the inter-disciplinary care team and makes psychosocial recommendations to assist in the resident/family’s overall care and well-being. - Documents in both the EMR and the required Care Plan sections of the MDS according to regulatory guidelines and timelines. - SW plays a key role in assessing for behavioral and mental health concerns and refers to the appropriate internal or external providers or services. - Advocates for the physical, emotional, social, educational and spiritual needs of the resident/family. - Provides casework, family sessions, crisis intervention and group work services to families and residents as needed. SW encourages family participation in care plan meetings and keeps family/legal guardian informed of child’s progress. - SW works closely with the child welfare system and assists the IDT when concerns of abuse/neglect arise. SW follows all protocols as a state mandated reporter. - In collaboration with IDT, SW oversees discharge planning process. SW confers with insurance case managers as needed to coordinate continued stays and discharge planning. - Keeps informed of community resources and makes appropriate referrals to support the psychosocial needs of both the resident and family. - Facilitates and encourages resident/family involvement in group and educational programs at the Pediatric Center. - Provides palliative care and end of life services to residents and families.   Job Summary   Provide speech therapy services to infants’ toddlers, preschool and school age children for an assigned caseload and conduct speech therapy assessments and evaluations. Featuring the largest long term residential medical center of its kind in the United States.   This is a Per Diem position - weekend coverage 11a-7p approximately 15 hours per week. $22.09 hour.   Per Diem Dietary Aide   Performs all duties related to sanitation of kitchen, dining and prep and storage areas. The employee is primarily responsible for the maintenance of sanitary conditions in the general kitchen area including but not limited to walls, floors and work surfaces. The employee will also assure that all kitchen equipment, tools used for preparation and service are cleaned and sanitized according to policies and procedures and within compliance of regulations set forth by State, Federal and Accrediting Agencies.   The employee also assists with service of meals to residents, including tray assembly, delivery and removal. The employee is primarily responsible for the accurate and efficient service of the children’s meals within compliance of their diet. The employee may also be required to perform or assist in other Health Care Dining Services activities to families, guests and other catered events.
