Responsibilities

Purpose of Position

 

To support the medical management of the residents in the Center with the completion of a comprehensive nutritional assessment.

 

Position Functions and Responsibilities

  • Collaborates with members of the health care team to provide medical nutrition therapy.
  • Assesses nutrition needs and identifies nutrient requirements according to age and disease specific needs of residents.
  • Identifies potential food-drug interactions according to system policy and provides education based on identified need.
  • Develops and implements the nutrition care plan for all assigned residents.
  • Foster continuity of care among all phases across the continuum of care.
  • Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy interventions.
  • Assesses patients’ learning needs, barriers to learning, preferred learning styles and educational level and provides nutrition education accordingly.
  • Provides nutrition education to patient and family members on appropriate food selection and/or preparation and eating behaviors to accomplish patient goals.
  • Write nutritional assessment reports for all residents upon admission, quarterly, annually, for significant changes, at discharge and as needed.
  • Completes admission, quarterly, annual and significant change, and discharge MDS documentation.
  • Participates in the management of the delivery of nutrition care services particularly to the assigned units.
  • Participates in performance improvement and research activities.
  • Supervises and mentors dietetic interns/students.
  • Actively assesses, evaluates and maintains professional development and competency.
  • Applies DOH and/or other regulatory standards in the provision of nutrition services.
  • Locates and reviews research findings for their application to optimally manage a disease or condition and improve patient outcomes.
  • Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator.

 

Qualifications

Education Requirements:

  • Bachelor’s Degree in Nutrition and Dietetics
  • Master’s Degree in Clinical Nutrition, preferred
  • Registered Dietitian with the Commission on Dietetic registration of the Academy of Nutrition & Dietetics

Experience

 

  • 1 to 2 years of position related experience
  • Pediatric and/or long-term care experience preferred

 

Salary Range:

$70,000/Yr. To $78,000/Yr.
